Referral intake
Start with a needs screening.
Select every area where support may be needed, identify the highest priority and tell us how soon help is needed. A navigator will review the complete picture and coordinate an appropriate next step.
Minimum necessary information
Please do not enter diagnoses, medications, Social Security numbers, medical records or detailed treatment histories.
Partners and professionals: You may submit on someone’s behalf only with their authorization or another legally permitted basis.

The initial request is limited to contact, broad need and consent.
Information is used to coordinate requested support and is not sold.
Consent can be limited or withdrawn, subject to legal and operational requirements.
Secure intake notice: Referral submissions are transmitted over encrypted HTTPS to a restricted 2 Steps Away Google Workspace referral log. Access is limited to authorized personnel and protected with organizational access controls and two-step verification. Please provide only the minimum information requested and do not submit medical records, Social Security numbers or detailed treatment histories.
